Midland's 6th Annual Golf Tournament will be held at Falcon’s Fire golf course (www.falconsfire.com). The Tournament will be held on February 22nd, 2008. Falcon’s Fire is located near all of Central Florida's amusement parks and attractions (Disney World, Universal, MGM, Sea World, etc.), so bring the whole family.
Registration Deadline is January 15th, 2008. Please complete this form and fax or email to De Carlson. If you need additional information about the Golf Tournament, Accommodations or the Peace Maker Foundation, please contact De Carlson directly. If paying by Credit Card, please complete our Authorization Form and send it to De Carlson. An additional $5 in Raffle Tickets will be provided if your Entry Fee is received by February 15th.
Golf Tournament Format: Each team will consist of 4 people, in which the 18 hole scramble scoring system will be used. Teammates will be provided if you do not have a complete foursome.

Shotgun Start: 8:00 AM, February 22nd, 2008
Contests: Longest Drive, Closest to the Pin, Putting, Hole in One contest.
Miscellaneous: In addition, their will be other on-course contests and a raffle drawing.
Entry Fee: The $75 per person entry fee is payable at Tournament and includes Green Fees, Golf Cart, Lunch, all contests and $10 in raffle tickets. All proceeds go to The Peace Maker Foundation. Rentals: You must call Falcon’s Fire’s Golf Shop ahead of time to reserve clubs or shoes prior to arrival. The cost is $55-$65. Golf Shop phone: (407) 239-5445.
The PARTY: There will be a Party at Bahama Breeze the night before the tournament on February 21st at 6:30 PM. Bahama Breeze is very close to the hotels and course: 8735 Vineland Ave, Orlando, FL 32836
